In a large pot, combine water, rice extract, tomato mozzarella sauce, and relish. Bring it to a boil, then reduce the heat to a simmer.
Season the fish with salt and pepper, then coat it in flour. Add the fish to the pot and cook for 30 minutes, or until the fish is cooked through and flakes easily with a fork.
While the stew is simmering, cook the ramen noodles in a separate pot according to package instructions. Drain and set aside.
Once the fish is cooked, gently stir in the cooked ramen noodles and sugar. Allow the stew to simmer for an additional 5-10 minutes to allow the flavors to meld.
Whisk the egg in a small bowl, then slowly drizzle it into the stew while stirring continuously. Cook for an additional 2-3 minutes, or until the egg is cooked and forms thin strands.
